Dear IPEG Members,

It has been three years since you were approached to join an IPEG study on virtual reality training. At that time the idea was too far out of reach and not a daily practice for many of us.

Meanwhile, many things have changed:

  • An educational committee has been institutionalized within IPEG to set out its goals concerning training in endoscopic surgery in children and infants;
  • There is increasing pressure from governments, patient societies and insurance companies for simulation training before residents and fellows are allowed to practice on patients;
  • Further professionalization of training modalities for residents and fellows in many countries;
  • New developments in simulation techniques.

At the 10th World Congress of Endoscopic Surgery and Joint 14th meeting of E.A.E.S. held September 2006 in Berlin, the results were presented from an international study, including many of the forerunners in virtual reality training, on "European Consensus on a Virtual Reality Training Program for Basic Endoscopic Surgical Psychomotor Skills; Acquiring Criterion Levels for Proficiency-Based Training". A training program was built in consensus by eight teams who all had personal experience in validating the LapSim® V.R. simulator. This was the largest series ever and included a group of 25 experts and a group of 60 novices in endoscopic surgery. The conclusion was that the parameters that proved to be useful for assessment can be used to define the training program endpoints. By doing so, a proficiency based training program can be offered to training centers that use this simulator for training basic skills in endoscopic surgery.

The IPEG Educational Committee would like to set up worldwide criteria for training in pediatric endoscopic training and could adapt this program and use the outcome to set the levels of proficiency based training in pediatric endoscopic surgery. This would enable us to have one single set of endpoints and compare the outcome of all participating centers worldwide through IPEG.
