The purpose of this agreement between site and IPEG is to outline the responsibilities of each party.

IPEG  is hereby loaning the Neonatal MIS trainers to the institution entered below (hereafter referred to as the “site”) for the purpose of participating in MIS data collection with IPEG and the other Neonatal MIS trainer sites for a period of 2 years.  The “site” acknowledges loan of Neonatal MIS trainers is contingent on the “site” meeting and maintaining the following conditions.  If the “site” fails to meet these conditions the Neonatal MIS Trainers will need to be returned to IPEG at the “site’s” expense.

Conditions:

1. The site has identified a pediatric surgeon who will oversee the Neonatal MIS Trainers project and data collection on performance, curriculum development, and outcomes associated with the incorporation of the trainers into the training programs for the next 2 years.
2. The site has the following equipment available to immediately begin Neonatal MIS Trainer use and data collection:
   1. Camera and monitor for use with Neonatal MIS trainers
   2. 3mm bowel grasper
   3. 3mm Maryland dissector
   4. 3mm scissors
   5. 3mm needle drivers
